This form is used to report safety concerns regarding elevators, escalators, amusement devices, aerial tramways, or other related equipment in the state of NC.

Accidents involving this equipment resulting in injury requiring more than first aid or damage to the device are required to be reported within 24 hours. Reports must be reported to the NCDOL Elevator and Amusement Device Bureau via telephone at 1-800-625-2267 or 919-707-7927.
