This is a limited 90-day use request application for permanent elevators used for building construction. Requirements are as follows:
- This elevator is to be used for the transportation of construction personnel, tools, and construction material only. It is not to be used to carry passengers, such as prospective tenants, rental agents, interior decorators, etc. It is not to be used to furnish the building with items such as desks, tables, beds, lamps, etc.
- This elevator is to be operated by a trained operator at all times. The operator shall be trained on the use of hoistway door unlocking devices (door keys) and the consequences of improper use of these devices.
- The limited certificate of operation is for a period not to exceed 90 days. The certificate may be renewed/extended at the discretion of the bureau chief upon receiving a written request showing justifiable cause for renewal. Such request must be received 15 days prior to the expiration of limited certificate.
The above terms and conditions will require signature of agreement at the time of your inspection. Only the PERMIT HOLDER can call for a NEW INSPECTION. The NEW inspection must be scheduled within 30 days of this approval. The EXTENSION REQUEST must be renewed 15 days prior to the date of the last inspection report. Each request requires this form (in writing) and arranged with inspector on each approval. The NEW inspection will be invoiced NET 30 days payable.
