Application to Install New Equipment
Complete one (1) application per unit. Acceptance subject to final field inspection. Inspection fee of $200 will be invoiced 30 days after final inspection.
In order to complete your application, please be aware of the following:
- Applications and Drawings must be emailed to dol.elevatormail@labor.nc.gov.
- For LULA applications, a letter of approval from the local building AHJ must be must be attached to your email and forwarded to dol.elevatormail@labor.nc.gov.
Applications that do not forward drawings and/or AHJ letters will not be processed.
