Safety Awards Application

New! As the Safety Awards Program prepares for the 2024 Safety Awards Banquet Season, please note that we have changed the way we will be processing the 2023 Safety Awards applications. 

The Safety Awards application process has transitioned from a hard copy mail-in form to an electronic online form. (Note: Please review the instructions and example application below before completing the online form. You may also want to print the BLS chart so you have it readily available when completing the application.) This new process will allow the application process to be more cost-effective and time-efficient for everyone involved. This also means that you will not receive a hard copy in the mail.

To ensure a smooth transition, we had sent an email to our current list of safety award recipients from the past several years on October 31, 2023. This email communication was to verify that the email address and contact information that we had on file for Safety Award applications was still correct. If you did not receive this email and would like to be added to our list, please email safety.awards@labor.nc.gov.

On December 21, another email was sent to those currently on our email distribution list with instructions for submitting your application online.
